Your personalised Bloom+ health report
A clear personalised health report summarising your assessment, results and agreed recommendations securely delivered within 72 hours of your appointment.
Follow-up support
After your assessment, you’ll have the opportunity to follow up by email once you’ve received your report, giving you time to ask any questions about your results or agreed next steps.

Time to talk through your general health, medical history, lifestyle and what matters most to you.

Your Bloom+ nurse will visit you at home for a relaxed, unhurried health assessment, with time to talk, complete your health measurements and discuss your results.
04 — RECEIVE
Within 72 hours, you’ll receive your personalised Bloom+ health report, bringing together your results and what was discussed during your appointment.
05 — FOLLOW UP
After receiving your report, you’ll have the opportunity to follow up by email with any questions about your assessment, results or agreed next steps
